Individual must possess a strong work ethic background and have a great positive attitude.

Primary/Essential Duties and
Responsibilities:
Introduce and enroll eligible population to the Enhanced Care Management Program Work with the Enhanced Care Management members who has multiple chronic conditions, high utilization of the ER and hospital and/or history of chronic homelessness Educate the members with multiple chronic conditions about evidence-based standards of care and self-management of their illnesses Collaborate with primary care providers and behavioral health providers to provide health and behavioral interventions that will maximize the Health Homes Program members' health outcomes; Conduct assessments of the members' history with medical/dental/behavioral health services, social and economic resources for purposes of developing a comprehensive care plan and providing linkage to services Document work through appropriate record keeping that follows GHC's policies and procedures Links the members to needed services and community resources Work with members and staff from other entities to coordinate discharge details for patients exiting care facilities (hospital, ED, treatment centers) Monitor the members' treatment adherence Offer services where patients live or congregate in order to increase access to care.
Facilitate integrated/collaborative case coordination meetings; participate in monthly administration/evaluation meetings Attend appropriate community resource meetings/trainings, as assigned
